Every Claim Form submitted requires a Unique Claimant ID. If you received a postcard or email notice regarding this settlement, your Unique Claimant ID was provided within that notice. If you have not received a postcard or email notice with a Unique Claimant ID, return to the "Home" page of the settlement website and click on “Registration". Enter your information to complete the registration process and you will then receive an email with your 13-digit ID, which you can then use to file an online claim. If you received an email notice directly from Amazon that included a “Unique Claimant ID” that is other than 13 digits starting with "83052", you must also register for a Unique Claimant ID from the “Home” page of the settlement website.

Only one Claim Form may be submitted per Unique Claimant ID. Only one Rock ‘N Play Sleeper can be claimed per submitted Claim Form and therefore, per Unique Claimant ID. If you bought or owned more than one Rock ‘N Play Sleeper for which you want to make claim, complete the “Registration” process and repeat for each additional Unique Claimant ID required.
