To receive the benefits you claimed on your initial Claim Form, you must complete the applicable sections of this Deficiency Cure Form according to the directions provided on your Claim Deficiency Notice and submit your Cure Form by June 12, 2026. Alternatively, you may download the paper Cure Form and submit it by mail so that it is postmarked on or before June 12, 2026.
The deadline to submit a timely deficiency cure form was June 12, 2026. You may still submit a deficiency cure form, but it will now be considered late. Late deficiency cure forms may or may not be considered as valid submissions.
Your Claim ID is required to access this online Cure Form. Your Claim ID was included on your Claim Deficiency Notice and all prior Class Notices you may have received.
